From the Latin lagena (flask, flagon, bottle) and forma (form, shape), in reference to the shape of the pseudobulbs (in his original description Bailey writes ‘pseudobulb shaped like a Florence flask’).
Plants forming small, dense clumps. Pseudobulbs moderately crowded, somewhat flattened, 7–10 × 6–10 mm, pale green, deeply grooved. Leaves erect, oblong, 50–100 × 5–8 mm, dark green, stiff. Racemes with thread-like stems 40–70 mm long, 1–4-flowered. Flowers bell-shaped, 6–7 × 3–5 mm, white, cream or pale green with pink labellum; flowers rarely wholly pale pink. Dorsal sepal hooded, 7–8.5 × 2–3 mm, irregularly warty. Lateral sepals triangular, 7.5–9 × 5–6 mm. Petals obliquely erect, 5–6 × 2 mm. Labellum c. 6 × 3.5 mm, thick, fleshy, curved, blunt.
Clumps small, compact; pseudobulbs flattish, grooved; leaves narrowly oblong; flowers bell-shaped, cream to pale green; dorsal sepal warty; labellum pink, smooth.
Qld (Mt Finnigan to headwaters of Tully River); 900–1,600 m altitude.
Locally common at high altitudes in rainforest; growing on shrubs, trees and rocks in shady situations where wind, clouds, mists and fogs are common.

First named Bulbophyllum lageniforme in 1904 by F.M. Bailey based on specimens collected in 1904 by Archibald Meston from Mt Bellenden Ker, north-eastern Qld.
